Florida Elks Children's Therapy Services HOW TO APPLY FOR SERVICES

The first step in being considered for admission to the program is to complete an application and return it to Florida Elks Children’s Therapy Services. To obtain an application call the program administrator at 1-800-523-1673 extension 241 and request an application. |
As soon as the completed application is received, it will be reviewed in order to determine if the therapy program is appropriate for the patient’s needs. The patient’s family will be advised whether or not the patient is accepted into the program. |
Eligibility for treatment will be based on a number of factors including medical and other criteria; such as but not limited to: |

• The patient must be a resident of Florida. • The patient must be between the ages of birth and 18 years. • The patient must have a condition necessitating Physical or Occupational therapies. • The patient must have financial need for free services. |
